Licensed Practical Nurse (LPN) - Long-term Care
Job Summary
Axiom Healthcare of Mount Vernon, a leading provider of healthcare services, is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team in Mount Vernon, Illinois. Responsibilities
Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians
Develop and implement individualized care plans, taking into account the unique needs and goals of each patient
Collaborate with other healthcare professionals, including RNs, therapists, and physicians, to ensure seamless and effective care delivery
Conduct routine vital sign checks, monitor patient conditions, and report any changes or concerns to the healthcare team
Maintain accurate and thorough records of patient care and treatment, including documentation and reporting of patient progress and outcomes
Participate in quality improvement initiatives and contribute to the development of new policies and procedures to enhance the quality of care
Demonstrate a commitment to patient-centered care, respecting the dignity and autonomy of each patient, and providing compassionate and empathetic support
