Community Mental Health Recovery Specialist

Horizon Health Services is seeking a Psychiatric Rehabilitation Specialist for its Lockport, NY clinic. The role involves supporting patients through outreach calls, telehealth, and in-person visits to identify barriers and coach engagement in treatment and healthy living.
