Case Manager

Humana is seeking a Case Manager to support members across Green Lake, Marquette, Adams, and Waushara counties. In this role, you will manage a regional caseload, conduct comprehensive health and psychosocial assessments, and create individualized care plans that connect members to the right medical, behavioral health, and community resources. You will coordinate care with providers, monitor outcomes, and adjust interventions to improve health, reduce avoidable utilization, and enhance quality of life. Working in a member-focused, collaborative culture, you’ll document all activities in case management systems, ensure regulatory compliance, and actively advocate for members.
