Social Work Services at the Mount Sinai Hospital
The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.
We are seeking a full-time Social Worker to provide comprehensive inpatient and outpatient social work services to patients receiving care through RMTI who are not on the transplant track . This role supports both the Liver Surgery and Liver Disease teams and offers the opportunity to work with a diverse patient population across the continuum of care.
ResponsibilitiesThe Social Worker will provide a broad range of psychosocial interventions, including support with adjustment to illness, complex medical decision-making, end-of-life care, discharge planning, and brief interventions to facilitate connection with substance use treatment and community resources. The position is particularly well suited for a clinician with strong communication and care coordination skills and an interest in working with individuals who use substances and patients with cancer.
Harm reduction and motivational interviewing are important components of this role, with an emphasis on engaging patients in a nonjudgmental, patient-centered manner and supporting them in identifying achievable goals related to their health and well-being.
This position participates in usual cross-coverage with the Transplant ICU Social Worker and, in addition to that cross-coverage, has one designated day per week assigned to provide coverage for the Transplant ICU Social Work role . Depending on caseload and inpatient census, the Social Worker may also be asked to provide additional coverage and support within transplant inpatient services and outpatient clinics.
There is also opportunity for program development , allowing the Social Worker to identify needs, develop new resources or workflows, and contribute to initiatives that enhance psychosocial care for patients with liver disease, substance use concerns and liver cancer.
The ideal candidate will bring strong clinical judgment, excellent communication and collaboration skills, effective discharge planning and care coordination abilities, and experience or a strong interest in working with patients with substance use concerns and or/with oncology patients.
QualificationsNYS LMSW or LCSW.
Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care both in and outpatient.
Excellent clinical and collaborative skills.
Organizational and program management skills.
Excellent presentation and writing skills.
Inpatient experience preferred.
Minimum one year in current position unless currently a float in Transplant/Oncology.

Compensation
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88691 - $101670.78 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
