Visit us online at: The Case Manager: provides quality case management and qualified professional support and services to inpatient social workers as assigned; works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.
