We are currently looking for an Occupational Therapist to join our team on a per diem basis. The position would be responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings within Monmouth County, NJ.

Position Summary:

Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings

Principal Responsibilities:

• Support organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician
• Document all changes in client's condition and updates care plans and outcomes accordingly
• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies
• Monitor quality and appropriateness of services provided
• Participate in VNAHG's QAPI and in-service programs
• Adhere to agency infection control and safety policies
• Adhere to the organization's policy in regards to absenteeism and appearance

Job Competencies & Minimum Qualifications :

• Occupational Therapy Licensure in NJ
• 2-3 years of prior clinical experience and community-based experience preferred
• Strong computer skills
• Strong communication and interpersonal skills
• Valid NJ drivers license

Working Conditions/Physical Demand : Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000 When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
