This is a per diem position. **Benefits*** Competitive salary* Focused one to one patient care* Medical, dental and vision insurance* Flexible work schedule & reasonable work hours* 401k matching* Supportive and friendly work environment***Requirements**** Minimum of 1 year of experience as a Registered Nurse with a valid license required* Current CPR/BLS Certification* Reliable transportation required* Experience with Kinnser strongly preferred* Comfortable driving to patient homes for patient visits* Prior experience with Palliative Care preferred* Prior experience in Home Health, Hospice, Infusion, or Home Care preferred**Responsibilities*** Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate.* Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.* Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s home or living facility.* Communicate necessary revisions to patient status and/or needs with the entire care team* Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.* Provide complete, accurate and timely documentation consistent with the plan of care #J-18808-Ljbffr
