Case Manager II - Waiver Case Management (Anoka County)People Incorporated is seeking Case Manager II to join our Waiver Case Management in Anoka County to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.Hours: Full-Time, Monday-Friday, 8am-4pmLocation: Anoka County (Hybrid position)Hiring Range: $24.25 - $25.75/ Hourly, based on qualificationsHiring Bonus: $1,000 to external candidatesPeople Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions.Main Job Duties:Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.Assessing client needs and ensuring continuity of care.Making ongoing referrals to resources based on assessment and current client need.Maintaining necessary daily records for continuity of service and reports to funding agencies.Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.Attending client monthly meetings to monitor services.Requirements for All Case Manager II positions:Valid driver’s license, excellent driving record and current auto insuranceEducation and Experience Required for Anoka County Waiver Case Manager II positions:Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field;ORBachelor’s Degree from an accredited four-year college with a major in any field, ANDOne year (2,000 hours) of experience as a social worker/case manager/care coordinator in a public or private social service agencyPreferred Education and Experience for Anoka County Waiver Case Manager II positions:Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, ANDOne year of experience in waiver case management or care coordination with adults experience severe mental illness.Benefits:Accrue up to 4 weeks of PTO/ESST, plus company-designated holidays and floating holidays403(b) Retirement Savings Plan with a 3% employer-matchMultiple health and dental insurance plan choices availableLifestyle Benefit – choice between company contribution to health savings account, student loan repayment assistance, or flex timeEmployer-paid Short & Long-Term Disability Insurance & Life InsuranceEligibility for state and federal loan forgiveness programsWe are committed to providing salary ranges for all open positions.
