Description

• Assess patients within either inpatient or outpatient settings and craft personalized treatment plans.
• Deliver physical therapy interventions with a focus on mobility, strength, balance, and overall function.
• Collaborate seamlessly with nurses, physicians, case managers, and other rehabilitation professionals to enhance outcomes.
• Inform patients and families about therapeutic goals, discharge planning, and home exercise programs.
• Record evaluations, treatment plans, and progress in alignment with facility and regulatory standards.
• Engage in care team rounds and interdisciplinary discussions to ensure cohesive care delivery.

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist .
• Preferred: CPR certification.
