Roles & Responsibilities

Provide individualized physical therapy assessments and treatments to residents
Develop and implement comprehensive care plans to restore mobility and function
Collaborate with the interdisciplinary care team to ensure optimal patient outcomes
Document patient progress and adjust treatment plans as needed
Educate patients and their families on therapeutic exercises and techniques
Contribute to a positive rehabilitation experience for every resident

Qualifications

Current Physical Therapist (PT) license in the state of Virginia
Doctor of Physical Therapy (DPT) degree from an accredited program
New graduates are warmly welcomed and encouraged to apply
Experience in skilled nursing or geriatric rehab is a plus
Excellent communication and collaboration skills

Education

Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

Strong knowledge of physical therapy principles and practices
Skill in patient assessment, treatment planning, and documentation
Ability to build rapport with elderly residents and their families
Familiarity with SNF regulations and care standards
Effective time management and organizational skills
