About the Role:

ANGELCARE HOME HEALTH SERVICES LLC is looking for a compassionate and skilled Physical Therapist to join our dedicated team in Vallejo, CA. Responsibilities:

Evaluate patients' physical conditions, functional limitations, and rehabilitation needs through comprehensive assessments

Develop and implement individualized physical therapy treatment plans to restore mobility and improve quality of life

Provide therapeutic exercises, manual therapy, and other evidence-based PT interventions in a home health setting

Monitor and document patient progress, adjusting treatment plans as clinically appropriate

Educate patients and caregivers on home exercise programs, safety techniques, and injury prevention

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care

Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards

Requirements:

Valid California Physical Therapist (PT) license in good standing

Graduate of an accredited Physical Therapy program (DPT or equivalent)

Home health or outpatient rehabilitation experience preferred

Strong knowledge of Medicare home health guidelines and documentation requirements

Excellent communication, interpersonal, and patient education skills

Reliable transportation and valid driver's license for home visit travel

CPR/BLS certification required or ability to obtain prior to start
