Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs and physician direction. This role includes delegation and direction of all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that allow for effective returns to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while ensuring compliance with organizational guidelines and maintaining professional standards.

Responsibilities

• Completes comprehensive evaluations of assigned patients and utilizes clinical reasoning to develop appropriate treatment plans.
• Collaborates with interdisciplinary teams to address barriers to discharge.
• Effectively manages individual, concurrent, and group treatment sessions based on patient care needs.
• Documents medical records clearly, concisely, and accurately in compliance with guidelines.
• Participates in student programs and provides a positive learning environment.
• Actively pursues professional development through continuing education and involvement in professional activities.
• Adheres to the discipline-specific State Practice Act and maintains professional licensure in good standing.
• Demonstrates awareness of organizational performance improvement initiatives and follows through with action plans to maximize performance improvement outcomes.

Required Experience / Certifications / Licensure

• BLS Certification required prior to first shift.
• Must hold a valid, current state license as a Physical Therapist.
• Prior experience in acute care and inpatient rehab settings preferred.
• Proficiency in EPIC (EMR) system is required.
