Job Description Physical Therapist position at Frederick Health Home Care in Frederick, MD. Full time opportunity with a busy, fast‑paced Home Health rehabilitation team providing Physical Therapy, Occupational Therapy, and Speech Language Pathology services to the community. Provide accurate and timely documentation for current patient caseload and oversee department compliance. Conduct patient and family education. Requirements Degree from an accredited school approved by CAPTE. Current state licensure or eligibility in Physical Therapy in Maryland. Minimum 1 year of Home Health experience; inpatient or skilled setting experience also considered. Basic Life Support certification through the American Heart Association. Strong skills in flexibility, time management, leadership, and communication. Focus on patient and customer service excellence. Experience with documentation utilizing EMR.
