Care Manager

Start: 10/26 - no days off within first two weeks Schedule: Monday–Friday, 9:00 AM – 5:30 PM EST 40 hours/week (Includes two paid 15-minute breaks and one unpaid 30-minute lunch) Equipment: Must have your own laptop (cannot be Chromebook or Macbook) Pay Rate based on Care Management Experience: 1 year experience: $21/hr 2 years experience: $22/hr 3 years experience: $23 4+ years experience: $24/hr The Care Manager supports care coordination efforts for New York Medicaid members by managing referrals, follow-ups, and member engagement. This role focuses on improving health outcomes through proactive outreach, documentation, and collaboration with providers and community partners.
