Role Summary: Provides comprehensive nursing care to residents in a skilled nursing facility, ensuring continuity of care across shifts. Supervises CNA, LPN, and support staff, participates in resident and family education, and advocates for residents in alignment with the facility’s mission. Key Responsibilities: Assess and document residents’ conditions, update care plans, and ensure compliance with documentation standards. Manage pain through assessment, education, and review of records. Develop and implement individualized care plans, evaluate outcomes, and revise as needed. Administer medications accurately, recognize clinical changes, and follow physician’s orders promptly. Communicate effectively during shift handovers, notify physicians of changes, and document appropriately. Address psychosocial, cultural, and spiritual needs, treating residents and families with respect and dignity. Lead and supervise healthcare team members, delegate tasks, and serve as a resource and preceptor. Act as a resident advocate, ensuring rights are protected, and remain calm in emergencies. Ensure compliance with regulations and facility policies, participate in ongoing professional growth. Qualifications & Skills: Graduate of an accredited nursing program with NYS RN licensure. Strong communication and interpersonal skills. Preferred: BSN degree and one year of long-term care experience. Basic Life Support certification is preferred. #J-18808-Ljbffr
