Evaluate patients in either an inpatient or outpatient setting and design individualized treatment plans

Administer physical therapy interventions focused on mobility, strength, balance, and function

Collaborate with nurses, physicians, case managers, and other rehab professionals to optimize outcomes

Educate patients and families on therapeutic goals, discharge planning, and home exercises

Document evaluations, treatments, and progress in accordance with facility and regulatory guidelines

Participate in care team rounds and interdisciplinary conferences to ensure coordinated care

Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited program

State licensure as a Physical Therapist

CPR certification preferred

Specialized certifications (e.g., neurological, geriatric) are a plus but not required

Previous experience strongly preferred

Ability to treat patients with complex medical and mobility issues

Strong communication and teamwork skills

Comfortable using digital charting and clinical documentation systems
