Support Coordinator: Field Visits & Care Plans Advocate

Public Partnerships | PPL

Public Partnerships LLC (PPL) is seeking a Support Coordinator to educate, guide, and assist Medicaid participants in self-directed care. You will visit participants in their homes (up to 60 miles) and help them manage care plans, hire direct care providers, and maintain safe living environments. You’ll split time between field visits and home-office documentation, collaborating with a dedicated team while using Microsoft Office, CRM tools, and electronic records to stay organized. #J-18808-Ljbffr Public Partnerships | PPL
