Case Manager-Adult Medical Day Program

(TCS) is seeking a Case Manager for Adult Medical Day Care Services (MDCSW) to support individuals with complex medical and functional needs. In this role, you will help clients receive quality care in community-based settings rather than institutional environments. Compensation & Benefits Base Salary: $60,000 Quarterly Bonus: Up to $1,250 based on performance Public Service Loan Forgiveness (PSLF) eligible employer Hybrid work environment (remote + local travel) Mileage reimbursement Benefits Package Includes: Medical (Kaiser), Dental and Vision (Mutual of Omaha) 401(k) with matching Paid Time Off (PTO) Flexible Spending Account (FSA) Company-paid Life, AD&D, and Short-Term Disability Flexible scheduling options Employee discounts Employee referral program (Bonus of $1,000) About the Role As a Case Manager – Medical Day Care Services, you will provide person-centered case management to individuals (age 16+) who meet nursing facility level of care but choose to live in the community. Key Responsibilities Conduct assessments using Person-Centered Planning (PCP) Develop and manage individualized service plans Coordinate Adult Medical Day Care and community-based services Document all service coordination activities in LTSS Maryland daily Conduct in-person client visits Monitor service effectiveness and client outcomes Ensure compliance with program and regulatory requirements Maintain accurate and timely documentation Qualifications Required: Bachelor’s degree in Human Services or related field (or approved equivalent) Some experience in case management or service coordination Strong communication, documentation, and organizational skills Valid driver’s license, reliable transportation, and safe driving record Ability to travel locally throughout Maryland Must be at least 21 years old Ability to pass background checks Authorized to work in the United States Preferred: Experience with Adult Medical Day Care or Medicaid Waiver programs Familiarity with LTSS Maryland system Experience working with individuals with complex medical or behavioral needs Schedule Full-time, Monday through Friday Flexible based on client needs About Total Care Services, Inc.
