Certified Diabetes Care Education Specialist (CDCES) - Hourly

Sea Mar Community Health Centers (FQHC), founded in 1978, is a community‑based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services to diverse communities, specializing in service to Latinos in Washington State. It is also a mandatory COVID‑19 and flu vaccine organization. We are recruiting for a full‑time Certified Diabetes Care Education Specialist (CDCES) located in Seattle – White Center. The CDCES delivers comprehensive services that bridge clinical and self‑management aspects of diabetes and endocrinology care, supporting and advocating for people affected by diabetes to optimize health outcomes. Responsibilities Provide comprehensive and seamless diabetes care and education to individuals across the lifespan. Deliver person‑centered care conducive to behavior change and improved quality of life. Advocate for patients and coordinate integrated clinical and self‑management services. Maintain complete and up‑to‑date charts, Misys EMREHR records, WIC/CIMS flow sheets, diet evaluation sheets, and other programmatic statistical records. Establish and maintain professional working relationships with team members and other Sea Mar staff. Serve as liaison between Nutrition/WIC and other clinic and community services, making referrals as appropriate. Assist with clerical and statistical program work as needed. Attend selected clinic, MSS, PHS Department, and WIC meetings; participate in continuing education as required. Act as a preceptor for dietetic interns, as appropriate. Execute all duties in accordance with organizational and departmental policies. Clinical Nutrition Therapy (MNT) Tasks Demonstrate knowledge and skills for age‑appropriate patient care. Identify and categorize patient age‑specific grouping needs (infant, adolescent, geriatric). Interview clients, review and document client history, anthropometric and laboratory data, and current eating habits. Use standard assessment tools and follow department policies for screening (e.g., weighing, measuring, hematocrits, growth grids). Maintain patient rights and confidentiality; document complaints or infractions. Provide program orientation, nutrition requirements, and scheduling assistance. Deliver individual and group nutrition education per program guidelines. Conduct medical, MSS, and patient nutritional assessment and counseling. Coordinate services with other health professionals and manage appointment follow‑up. Document assessments, treatment plans, and counseling in the medical record. Education and Qualifications Hold a Bachelor’s or Master’s degree in nutrition from an accredited university. Completed an accredited dietetic internship program or coordinated undergraduate program in dietetics. Completed 7 hours of HIV/AIDS coursework. Hold Washington State registration and certification for Dietitian or RD/CD; or obtain these credentials within the period specified in Sea Mar Nutrition Program Policies. For WIC staff, possess Medical Assistant‑Registered Certification (required for hemoglobin testing). Demonstrated interest in nutrition services and education; experience in community nutrition settings desired. English/Spanish bilingualism preferred but not required.
