Position Summary

The Home and Community-Based Mental Health Paraprofessional provides direct support services to children, adolescents, and/or adults with mental health, behavioral health, or co-occurring needs in home and community settings. Services are delivered in accordance with North Carolina service definitions and under the supervision of a Qualified Professional (QP). The Paraprofessional assists individuals in developing coping skills, daily living skills, social skills, and behavioral management strategies to promote stability, independence, and community integration.

Key Responsibilities

• Deliver skill-building interventions outlined in the Individualized Service Plan (ISP) under QP supervision.
• Provide one-on-one support in home and community settings.
• Assist individuals in developing coping skills, emotional regulation, and problem-solving skills.
• Support behavioral interventions and reinforce positive behavior strategies.
• Model appropriate social interactions and daily living skills.
• Assist with crisis prevention and follow agency crisis protocols as needed.
• Document services accurately and timely in accordance with North Carolina Department of Health and Human Services (NCDHHS) guidelines and agency policies.
• Maintain communication with supervisors, caregivers, and treatment team members.
• Participate in team meetings, supervision sessions, and required trainings.
• Maintain confidentiality in compliance with HIPAA and state regulations.

Minimum Qualifications

• High school diploma or GED required.
• At least 1–2 years of experience working with individuals with mental health, developmental, or behavioral health needs preferred.
• Must meet North Carolina Medicaid and service definition requirements for paraprofessional staff.
• Valid North Carolina driver’s license, reliable transportation, and current auto insurance.
• Ability to pass background check, drug screening, and credentialing requirements.

Preferred Qualifications

• Associate degree or coursework in human services, psychology, social work, or related field.
• Experience providing services under Intensive In-Home (IIH), Community Support Team (CST), or other NC Medicaid mental health services.
• Knowledge of trauma-informed care, mental health and IDD population and evidence-based behavioral interventions.

Work Environment

• Services are primarily delivered in client homes and community locations.
• Requires travel within the assigned service area.
• Flexible schedule, including evenings and occasional weekends as needed.

Physical Requirements

• Ability to sit, stand, bend, and move for extended periods.
• Ability to safely respond to behavioral crises when necessary (following agency training).

Supervision

This position receives supervision from a Qualified Professional (QP) in accordance with North Carolina Medicaid service definitions and agency policies.
