UAS Assessment Manager (RN), Contract-to-Hire, Monday-Friday schedule

FlexStaff is seeking a Registered Nurse with experience in LHCSA to work as a UAS Assessments Manager for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area. The area of responsibility will be in Bronx, NY and Westchester, NY.

  • Schedule: Monday - Friday 8:30am-5:00pm (1 hour lunch) (FT). Weekly Hours: 37.5

  • Pay rate: $73 per hour

  • Location: Bronx, NY

  • Temporary contract with potential to be hired permanently

In this role you will be conducting in-home clinical assessment process, including but not limited to clinical onboarding and reassessment, UAS-NY completion, comprehensive assessment, medication review, and post-hospital discharge evaluation.

Requirements:

  • Degree from an NLN accredited nursing school (RN, BSN required or at least five (5+) years' experience in health care setting).

  • Current active and unrestricted license and registration in New York State as a Registered Nurse required.

  • Minimum of two (2+) years of experience in long-term home health care (LHCSA).

Responsibilities include, but not limited to:

  • Work closely with intake and enrollment and care delivery teams regarding in-home or virtual assessment appointments with prospective or enrolled participants (utilizing the Uniform Assessment System of New York, UAS-NY). During this visit, answers questions about the PACE program and emphasizes the unique benefits of the PACE program.

  • Conduct in-home assessments for PACE participants utilizing the UAS-NY to determine initial or continued plan eligibility in accordance with the requirement need of Community Based Long Term Care Long Term Care

  • Maintain a schedule of 2-3 UAS‑NY assessments per day, as needed.

  • Ensure all changes are accurately documented in the EMR, with narrative comments and supporting documentation; escalate significant changes to Care Delivery.

  • Review and update primary diagnoses and medications at key intervals to align EMR and UAS; adjust codes/med lists as needed and escalate issues to ensure proper care management.

  • Promote and educate referral sources about the PACE program as needed.

  • Educate potential/new PACE participants on plan benefits, services, and the care delivery model.

  • Escalate assessment questions/concerns to the quality review team to ensure proper completion of UAS‑NY and timely intervention for on‑site issues.

  • Complete and/or reassess the PCAT Tasking Tool to determine appropriate ADL/IADL home assistance; recommend modifications and escalate as needed.

  • Develop proposed service plans and obtain required documentation during home visits.

  • Utilize Lenavi analyzer during reviews/assessments.

  • Document all interactions and outcomes in the clinical/electronic record system.

  • Complete assessments, notes, service plans, and documentation within 48 hours.

  • Collaborate with the quality review team to ensure accuracy and completeness in the Health Commerce System and EMR.

  • Maintain knowledge of applicable CMS/PACE guidelines and organizational policies; ensure timely completion and documentation of UAS‑NY.

  • Remain responsive and available by phone/email.

  • Able to work some weekends and travel across boroughs.

  • Provide key clinical information to Care Delivery prior to enrollment to support smooth transitions for participants with complex conditions.

  • Partner with Care Delivery and the IDT to share updates, address concerns, and improve processes.

  • Serve as the clinical point person for post‑enrollment questions or issues.

  • Participate in continuous quality improvement to enhance staff performance and outcomes.

  • Advise on operational improvements (turnaround, scheduling, cost efficiencies, engagement, reporting).

  • Review key performance metrics to improve workflow and quality.

  • Other duties as assigned.

When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g. location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget, and internal equity).

The salary range for this position is $57.00-$57.00/hour
