PT - Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients, developing, implementing, and modifying treatment plans in accordance with the patients’ needs, goals, and physician direction. Responsibilities include delegating and directing discipline-specific therapy services, ensuring effective functional treatment programs to facilitate a return to community activities while serving as an educator for patients, families, and coworkers. Responsibilities Complete comprehensive evaluations of assigned patients. Utilize clinical reasoning to develop appropriate treatment plans and goals. Ensure medical record documentation is clear, concise, accurate, and timely. Participate in the student program as assigned, providing supervision and evaluations. Create individualized professional development plans and pursue continuing education. Adhere to discipline‑specific state practice acts and maintain professional licensure. Demonstrate awareness of organizational performance improvement initiatives. Required Experience / Certifications / Licensure Degree in Physical Therapy from an accredited school. 1+ year of outpatient clinical experience. Current CPR certification. Active license for the state where work will be performed. Schedule This position involves a 5x8.0 shift schedule. Assignment Duration This is a 13‑week contract position.
