Family Medicine Physician

Reporting to the Medical Director, is responsible for ensuring coordinated and integrated care. Will examine and treat patients with a wide range of conditions and refer those that need a higher level of care to a specialist or appropriate facility. Typical job duties include taking patient histories, performing or requesting diagnostic tests, making diagnoses, prescribing treatment, administering vaccinations and performing follow-up examinations, as well as educating patients in disease prevention and health maintenance. Additional duties include preparing medical records, clinical reports and correspondence. Commit to train future family physicians to deliver quality health services to patients of diverse cultural, ethnic, and economic backgrounds.

• Experience in primary care setting preferred.
• Ability to work as part of a multidisciplinary team.
• Interest in serving low income-multicultural populations.
• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.
• Demonstrates knowledge of the principles of growth and development over the life span.
• Ability to identify and categorize each patient's age specific grouping of needs, such as those for infant, adolescent or geriatric patients.
• The ability to establish and maintain collegial relationships with coworkers, and to function in a calm manner during stressful and emergency situations.
• Demonstrates flexibility and resilience in response to changing clinical or administrative environments.
• Must have valid driver's license from State of residence. Must be able to transport oneself within the service area.

Major Duties & Responsibilities:

• Provide quality care to patients according to evidence based medical protocols and funding source guidelines.
• Initiate an appropriate assessment of patient's health status, ranging from focused to comprehensive, depending on patient need.
• Provide and manage comprehensive medical care, including diagnosis and nonsurgical treatment of diseases, common health problems, such as infections, influenza and pneumonia, as well as serious, chronic, and complex illnesses for patients of all ages.
• Monitor patients' conditions and progress and reevaluate treatments as necessary.
• Explain procedures and discuss test results or prescribed treatments with patients.
• Advise patients concerning diet, activity, hygiene, and disease prevention.
• Collect, record, and maintain patient information, such as medical history, reports, and examination results and prepares reports and correspondence for referrals.
• Refer patient to medical specialist or another practitioner when necessary.
• Prescribe medication in accordance with CT statute and professional practice guidelines.
• Participate in formulating, interpreting, implementing, and evaluating policies, procedures and workflows.
• Assist with quality improvement activities as required and participates in Quality Assurance and Utilization Reviews.
• Participate in Accountable Care Organization activities as required.
• Perform diagnostic tests and procedures and therapeutic procedures, within the scope of practice, including but not limited to intramuscular and subcutaneous injections; electro-cardiogram; application of dressings and bandages; administration of medications; cardiopulmonary resuscitation; auditory screening; visual screening; and aseptic techniques.
• Analyze accumulated health information and utilize data to make clinical decisions.
• Interview, historical, physical examination, and diagnostic data, modifying treatment as appropriate.
• Have a working knowledge of ICD10, CPT and HCPCS coding and managed care, and work collaboratively with the billing for training as necessary.
• Maintain and complete patients' health records or charts legibly, concisely and close charts per company policy.
• Work as an integral part of the center's clinic clinical team and work at other sites as needed.
• Participate in the on-call rotation schedule as required.
• Comply with the regulations and policies required of a Federally Qualified Health Center.
• Complete administrative assignments as requested by Medical Director and/or Center's administration.
• Perform other duties as assigned to support the Center's Mission, Vision, and Values.

• Professional positive attitude, understanding of customer service principles, trustworthiness and excellent interpersonal skills.

Job Qualifications/Requirements:

• Education: M.D./D.O. degree from an accredited school. Documented completion of residency from an accredited institution in an appropriate specialty. Board certification or Board eligibility desirable. Current licensure in the State of Connecticut. Valid Federal and State Controlled Substance Licenses.
• Experience: Responds constructively to pressure situations and time limits. Able to accept constructive criticism and instruction while maintaining a positive attitude towards patients, managers, programs and job responsibilities. Acts as a role model willing to accept or volunteer on projects as opportunity arises.

Salary Range for the position: $198,000.00 to $240,000.00
