Hospice RN Case Manager: Compassionate Care Leader
The Care Team Home Health & Hospice
The Care Team Hospice is seeking a compassionate Hospice RN Case Manager in Peoria, IL. You will coordinate patient care, develop individualized plans of care, and collaborate with physicians and the interdisciplinary team to ensure comfort and quality of life for patients at the end of life. Monday–Friday, 8:00 AM–4:30 PM with on-call opportunities and compensation. Prior hospice nursing experience and HCHB proficiency are preferred. #J-18808-Ljbffr
