Home Health Nurse (On-Call) — Care Plans & Wound Care

Phaxis seeks an experienced RN to provide in-home health nursing care in the Albany, KY area. The role includes assessing patients, developing and implementing care plans, and coordinating with a multidisciplinary team to support health and safety in community settings. The RN will supervise plan implementation, monitor medications, and ensure documentation is up to date, while working a Monday–Friday schedule with on-call duties. #J-18808-Ljbffr Phaxis
