Patient Demographics: Geriatric/frail elderly
Patient Volume: TBD
Admissions: As Needed
Rounding included: Yes
Phone Consults: As Needed
Case Mix: Primary care, chronic disease management, preventive care, and transitional care



FACILITY INFORMATION
Collaborative environment with physicians, nursing, therapy, and support staff

REQUIRED PROCEDURES/DUTIES
Comprehensive geriatric assessments
Chronic disease management
Preventive care screenings
Medication management
Transitional care and discharge planning

JOB REQUIREMENTS
Active NC License
NCCPA certification
DEA
BLS
Clean Record (highly preferred)
2 years PA experience
Experience with geriatric or frail elderly populations (preferred)
