Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to join our Healthcare & Medical Services team. In this role, you will complete comprehensive hospice admissions, conduct clinical assessments, verify eligibility, and develop initial plans of care. You will educate patients and families on hospice services, coordinate with physicians and the interdisciplinary team, and ensure timely, accurate documentation in the EMR.
