It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.The Caregiver is responsible for assisting participants with all aspects of personal care activities and implementing strategies for promoting increased levels of self-care when possible. Responsible for assisting participants with Instrumental activities of daily living and implementing strategies for promoting increased levels of independence when possible.
