The Weekend Admissions Nurse (RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and completing clinical documentation. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Your services help clients stay in their home, maintain a comfortable lifestyle, and live happier lives.Pay range: $41.00 - $46.00/hourWeekly Hours: 24 (.6 FTE)Schedule: 8:00 am to 5:00 pm, Friday - SundayPosition Benefits:Competitive Compensation + Travel Pay401k plan w/match- Traditional & Roth- IMMEDIATE VESTING, PTOMedical, Dental, Vision, HSA, Short Term DisabilityAwesome Company Culture + Work Life Balance!Stable & Growing CompanyDuties & Responsibilities:Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and RecertificationsAbility to drive throughout the twin cities metro for visits and clientsDevelop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of condition.Facilitate referrals to interdisciplinary teams (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention).Perform skilled nursing tasks as assigned in coordination with the Clinical Manager and CareAparent scheduling.Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed care team members.Prepare visit/shift reports, patient records summaries, and lead care team case conferences with the care team.Report on changes in client condition to physicians and Clinical Manager.Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms.Attend monthly in-person or virtual nurse meetings with the Clinical TeamQualifications:Current Minnesota RN licensePrevious experience with seniors and client assessmentsExcellent customer relations and interpersonal skillsExcellent verbal/written communication skillsOk with travel to and from client homesAbility to work on call for some holidaysPossess a valid driver's license and insured personal vehicleCareAparent is an Equal Opportunity Employer, and qualified applicants will not be discriminated against based on race, color, religion, gender, national origin, disability, or veteran status.CareAparent participates in the federal E-Verify program. #J-18808-Ljbffr CareAparent
