Growing together.As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker.Primary Responsibilities:Performs patient assessments and collaborates with the care team to develop and implement a plan of careMakes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing soPromotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniquesSupervises LPNs and HHAsCompletes documentation timely and accuratelyRegularly communicate patient progress to the clinical manager and care teamPlans and provides staff educationPerforms on-call responsibilities and on-call services to patients/families as assignedParticipates in clinical development and continuing education programsOther duties as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current RN license, specific to the state(s) you are assigned to workValid driver's license, reliable transportation and liability insuranceCurrent CPR certification1+ years of clinical experience as a Registered Nurse (RN)Pay Range $71,677 - $107,516 annual total cash target pay $41.35 - $62.03 per visit point $34.46 - $51.69 hourly rateAt UnitedHealth Group, our mission is to help people live healthier lives and help make the health system work better for everyone.
