Document discharge notes and coordinate discharge care, including medication reconciliation, follow-up appointments, consultations, and coordination with other team members. Order and interpret diagnostic tests and therapeutic procedures, including lab tests, ECGs, radiology studies, and oxygen therapy based on current treatment guidelines. Respond to medical emergencies for Community Living Center patients and coordinate transfer to a higher level of care when necessary. Order VA specialty service consults and initiate non-VA medical care consults according to VHA policy and local facility availability. Order interfacility transfers and transportation for Community Living Center Veterans. Order prescribed devices, supplies, durable medical equipment, and medications for patient care. Conduct goals-of-care conversations with high-risk patients regarding life-sustaining treatment limitations and choice of surrogate. Enter life-sustaining treatment progress notes and corresponding orders. Provide clinical oversight and participate in peer review of other Community Living Center providers through chart reviews as needed. Serve as surrogate coverage for absent Community Living Center providers, including review of electronic medical record alerts and other assigned duties. Participate in team coverage for vacations, sick days, and other absences. Attend provider meetings, departmental meetings, and medical staff meetings as requested. Participate in formulating multidisciplinary treatment plans. Provide health education to patients and other team members. Coordinate with the Community Living Center admissions team to determine appropriateness for admissions and discharges. Refer patients to appropriate VA or community programs. Perform other necessary duties assigned by the supervisor within the provider's scope of practice. Basic Qualifications: Must be a U.S. Citizen. Non-citizens may only be appointed when qualified citizens cannot be recruited in accordance with VA policy. Must be proficient in written and spoken English. Must have a master's degree in any discipline, or a bachelor's degree in any discipline with one year of experience as a Physician Assistant. Must have successfully passed, or be in the process of taking, the Physician Assistant National Certifying Exam. Must currently hold, or will hold, a full and unrestricted license to practice in a U.S. State, Territory, Commonwealth, or the District of Columbia. Applicants pending completion of educational, certification, or licensure requirements may be referred and tentatively selected but may not be hired until requirements are met. Grade Determinations: Physician Assistant-I: No experience or education beyond the basic requirements. Physician Assistant-II: Requires progressively increasing responsibility and experience, such as two years of successful PA practice, successful completion of an accredited PA post-graduate residency program, or a doctoral degree in a health-related field. Physician Assistant-III: Full performance level. Requires advanced independent practice, such as three years of successful PA practice; completion of a PA post-graduate residency plus one year of professional practice; or a health-related doctoral degree plus one year of professional practice. The full performance level of this vacancy is PA-III. The actual grade selected may range from PA-I to PA-III. Education: Master's degree in any discipline, or bachelor's degree in any discipline plus one year of Physician Assistant experience, is required under the PA basic requirements. Education must be from an institution recognized by the U.S. Department of Education. Foreign education must be supported by a certificate of foreign equivalency if used to meet qualification requirements.
