Mederi Care Tenders, a part of LHC Group, is seeking a Physical Therapist for Home Health. Candidates will assess patient functional status, develop treatment plans, deliver interventions, and document progress. Primary Responsibilities: Evaluate patients' functional status and physical therapy needs, assisting the physician in developing the therapy plan of care. Provide services within the scope of practice defined by state laws, coordinating with other health care team members. Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician. Treat patients to relieve pain, restore function, and maintain maximum functioning using acceptable standards of practice. Instruct and inform patients, family/caregivers, and health care team members regarding the plan of care, functional limitations, and progress toward goals. Visit patients per Plan of Treatment, complete a progress note for each visit, and submit notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state-specific guidelines. Report outcomes of evaluation, goals, and projected frequency of care. Required Qualifications: Current Physical Therapy licensure in the state of IL. Current CPR certification. Current driver's license and vehicle insurance, or access to a dependable vehicle or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Annual Target Pay: $91,165 - $136,747. Per Visit Point Pay: $43.83 - $65.74. Hourly Pay: $52.60 - $78.89. Compensation also includes benefits such as comprehensive benefits package, recognition programs, equity stock purchase, and 401(k) contribution. All amounts are based on local labor markets, education, work experience and may increase over time based on productivity and performance.
