Role Qualifications:

• Evaluate and treat patients with individualized, evidence-based plans of care
• Document evaluations, treatment goals, progress notes, and discharge plans clearly and timely
• Educate patients and families to support recovery and long-term wellness
• Collaborate with providers, team members, and community partners
• Assist in maintaining departmental policies and contributing to in-service training

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
• Current Physical Therapy license (or eligibility) in the State of Michigan
• Active CPR certification
• Strong communication and organizational skills
• Ability to lift or carry up to 60 pounds frequently and up to 100 pounds occasionally
