Clinical Pharmacist Specialist, Ambulatory - Pediatric Pulmonary & Allergy

SUMMARY Reports to the Director of Specialty Pharmacy, or Ambulatory Manager as applicable. Under the supervision of the Pharmacist‑In‑Charge, Senior Clinical Pharmacist Specialist, or Clinical Coordinator, coordinates pharmacy clinical programs including pharmacotherapeutics, pharmaceutical care, drug information services, and pharmacoeconomics. Collaborates with multidisciplinary care providers to monitor, assess, recommend changes, and optimize medication use in therapeutic specialties. Serves as a subject‑matter expert and real‑time go‑to resource in specialty areas. Participates in developing therapeutic drug policy and implements corrective strategies to contain drug costs within patient settings and therapeutic classes. Monitors pharmacy clinical services through continuous quality improvement activities to ensure customer expectations are met or exceeded. Provides input for developing and implementing policies, procedures, and budgets for the clinical practice setting. RESPONSIBILITIES Coordinates and oversees new and established pharmacy clinical services within scope of practice; establishes priorities for effective patient care and clinical pharmacy services within departmental and organizational policies. Provides patient care and education through comprehensive medication reconciliations, medication‑related patient counseling, plan‑of‑care adherence counseling and support, and general health education coaching. Emphasizes safe and effective use of medications, identifies potential drug interactions, and performs related activities. Facilitates provision of resources to maximize medication therapy success with consideration of patients' social determinants of health. Coaches professional staff (home medical team, other practitioners, providers) on pharmaceutical care and pharmacoeconomic principles, facilitating safe and cost‑effective medication use within the integrated care delivery model. Provides support for patients enrolled in the Brown University Health Specialty Drug Patient Management Program (SDPMP) and those covered by Brown University Health‑affiliated accountable care organizations. Appropriately documents patient encounters, interventions, and plans of care in the electronic health record (EHR). Participates in the development, implementation, and integration of patient‑focused medication therapy management and pharmaceutical care services. Reviews and analyzes proposed clinical services, recommending and implementing changes, allocation of resources, and technologies to increase efficiency and level of pharmaceutical care. Implements, interprets, and provides consultation for policies, procedures, and standards of practice related to medication use. When practicing under a collaborative practice agreement, follows practices consistent with services delineated in the agreement and ensures compliance with existing state and federal regulations. Ensures pharmacy clinical services and programs support established standards of care; meet or exceed patient care quality standards and consider patient developmental stages (neonate, child, adolescent, adult, geriatric). Maintains current professional competence in the specialty area; participates in ongoing independent study, professional organizations, and continuing education activities. Communicates effectively with staff, management, and customers to facilitate timely flow of drug information; provides accurate written and verbal communication. Works with Pharmacy and Therapeutics Committee to establish drug therapy management protocols and collaborative practice; monitors targeted drugs/therapeutic classes and implements corrective strategies. Participates in quality management and improvement activities, practice performance improvement initiatives, and quality measures; identifies opportunities for improvement and leads or participates in relevant committees and task forces. Participates in development of a strategic vision for the department; contributes to short‑ and long‑term goals aligned with Brown University Health mission and values. Provides input to assist in effective drug budgeting and allocation of resources through application of pharmacoeconomic principles to achieve defined goals. Participates in determining qualifications and competence of department personnel; develops tools and standards to establish and improve staff competency levels in therapeutic specialty. Participates in orientation of new employees, in‑service training, and continuing education programs. Provides education and training to pharmacy residents, interns, nurses, physician assistants, medical students, and other residents; serves as preceptor, completing competency documentation and providing oversight for pharmacy internship rotations. Presents formal lectures to pharmacists, health professionals, and the lay public; supports department academic affiliations, adjunct clinical faculty roles, and lectures for colleges of pharmacy, nursing, and physician assistant programs. Conducts pharmacy practice, clinical, and data‑related research; authors abstracts, scholarly papers, and publications in accordance with departmental policies. Performs immunizations and/or medication administrations per Massachusetts or Rhode Island pharmacy board rules, depending on position responsibilities. Performs other duties as assigned. BASIC KNOWLEDGE Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited college of pharmacy. Current unrestricted licensure as a registered pharmacist in the Commonwealth of Massachusetts, or licensed pharmacist in Rhode Island (or 90‑day temporary pharmacist license pending full licensure) prior to expiration of the 90‑day license. Within six months of start date, possess and maintain an American Council for Pharmacy Education (ACPE) approved Immunization Certification and American Heart Association Basic Life Support (BLS) Certification if performing immunizations or medication administrations. EXPERIENCE Successful completion of Post Graduate Year 2 (PGY‑2) residency in an accredited program in the specialty area and national board certification in that area within one year of hire. Alternatively, two years of clinical experience, knowledge, and development may be considered; requires national board certification in the specialty area or Pharmacotherapy within one year of meeting board certification eligibility. Effective interpersonal and communication skills, both written and verbal, to interact appropriately with colleagues and others. Effective organizational and presentation skills.
