Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver skilled physical therapy interventions to support patient recovery and functional goals
• Monitor and document patient progress, adjusting treatment plans as appropriate
• Collaborate with interdisciplinary care team members to ensure coordinated patient care
• Maintain valid state-issued photo identification and wear a photo badge at all times while on site

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Hampshire
• Graduate of an accredited Physical Therapy program
• Valid state or federal issued photo identification required for facility access

Preferred Experience / Skills

• Prior experience in a contract or travel therapy setting
• Strong documentation and communication skills
