HIV Care Case Manager I - Patient Support

CAN Community Health in Miami area is hiring a Medical Case Manager I to coordinate comprehensive case management services and ensure access to care. You will screen eligibility, verify insurance, develop care plans, and work with the care team to improve health outcomes. The role requires a Bachelor’s degree in a related field and at least one year of HIV prevention/intervention experience. A valid driver’s license is required.
