About the Role

We are seeking a licensed Physical Therapist to join our team in Lakeway, Texas. As a Physical Therapist for Vista Ridge Senior Care, you will be responsible for providing high-quality physical therapy services to our residents in a skilled nursing and rehabilitation setting. You will work closely with our multidisciplinary team to develop and implement individualized treatment plans that promote optimal physical function, mobility, and independence for our residents.

Responsibilities

• Conduct thorough evaluations to identify residents' physical therapy needs and develop individualized treatment plans to address those needs

• Implement and modify treatment plans as necessary to achieve optimal outcomes

• Deliver high-quality physical therapy services, including exercise programs, manual therapy, and modalities

• Communicate effectively with residents, families, and healthcare teams to ensure continuity of care and achieve optimal outcomes

• Maintain accurate and up-to-date records of patient progress and treatment plans

• Participate in quality improvement initiatives to improve patient outcomes and reduce readmissions

Requirements- Physical Therapist (Full-time) :

• This is a Physical Therapist (Full-time) position that requires a degree in Physical Therapy from an accredited program

• Licensure as a Physical Therapist in the state of Texas

• Physical Therapy experience in a skilled nursing and rehabilitation setting
