This role is key in helping residents regain mobility, strength, and independence.Key ResponsibilitiesProvide therapy treatments as directed by the Physical TherapistFollow physician treatment plans and established care goalsDocument daily treatments and weekly progress notes per regulatory requirementsCommunicate resident progress, concerns, and treatment plans with the care teamParticipate in care planning and rehabilitation conferencesAssist with discharge planning and caregiver educationTrain restorative aides and support nursing staff as neededMaintain clean, safe therapy areas and equipmentEnsure compliance with state practice act and facility policiesQualificationsCurrent PTA license in South Carolina (required, in good standing)Graduate of an accredited Physical Therapist Assistant programExperience in skilled nursing or rehab setting preferredFamiliarity with PointClickCare and Casamba preferredStrong clinical, documentation, and communication skillsWe are an equal opportunity employer.
