Nurse Case Manager - Registered Nurse

We are seeking a Registered Nurse to service patients throughout the Grandstrand area. The Nurse Case Manager plays a critical role in the care team by visiting patients, facilitating provider access, traveling to patients’ homes and facilities, evaluating patients, and initiating telemedicine visits. The position requires adherence to all applicable laws, regulations, and policies within the scope of practice. Full‑time, salary‑based, 8‑hour shifts Monday‑Friday (8 am–5 pm). A successful Nurse Case Manager will perform the following essential duties. Area of Responsibility Facilitate receipt of all medical records from the patient’s primary provider and specialists. Review medical records. Complete consents with patients. Enroll patients in Care Management if eligible. Initiate a Care Management Plan of Care for eligible patients. Capture diagnoses at the highest specificity and ensure acceptance. Complete AWVs to be reviewed by the provider. Conduct cognitive impairment screenings. Administer Social Determinants of Health (SDoH) assessments. Complete ACPs to be reviewed with the patient by the provider. Evaluate for home health, hospice, palliative care, or consults with Your Health Specialty Division. Evaluate for RPM devices, resources, or tools that may improve patient quality of life. Communicate and coordinate care. Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments. Assess for adaptive equipment and DME. Ensure safe environment. Evaluate acute conditions or follow‑up from previous visit. Document and log Care Management activities accurately; work with the care team to keep plans up to date. Qualifications Licensed nurse; Registered nurse preferred; license in good standing. Minimum of three (3) years clinical experience preferred. Experience in community settings preferred. Proven communication and collaboration skills with interdisciplinary teams, patients, and caregivers. Strong written and verbal skills. Basic computer knowledge. Leadership and interpersonal skills. Team player capable of managing multiple responsibilities with sound judgment. Flexible hours and travel between offices and facilities.
