The Registered Nurse (RN) Case Manager - Home Care contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Registered Nurse (RN) Case Manager - Home Care conducts nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Registered Nurse (RN) Case Manager - Home Care for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Registered Nurse (RN) Case Manager - Home Care is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies. Qualifications for a Registered Nurse (RN) Case Manager - Home Care: Current unencumbered RN license in Rhode Island. Minimum of one year nursing experience, home care experience preferred.
