Job Responsibilities

Monday - Friday (Standard Business Hours)
Oversee Daily Intake Operations Including Referrals, Admissions, and Patient Onboarding
Supervise and Lead Intake Staff to Ensure Timely Start of Care
Manage Intake Logs, Reports, and Patient Flow Tracking
Communicate with Referral Sources to Clarify Patient
Needs and Coordinate Care
Collaborate with Branch Directors and Business Development Teams
Ensure Accurate and Timely Data Entry into EMR Systems
Monitor Compliance with PDGM Guidelines and Documentation Standards
Assist with Strategic Planning, Budgeting, and Process Improvement Initiatives
Participate in Quality Assurance and Performance Improvement Activities
Act as a Liaison Between Intake, Clinical Teams, and Executive Leadership

Job Requirements

Experience in Home Health Required
Leadership or Supervisory Experience Required
Strong Knowledge of Intake Processes, Referrals, and Admissions
Experience with EMR Systems and Healthcare Documentation Standards
Familiarity with PDGM Guidelines Preferred
Strong Organizational and Communication Skills
Ability to Thrive in a Fast-Paced Environment
