Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 1 year of relevant experience required License: Valid State License (Compact license must be in hand at submission) Certifications: BLS, MSW Must-Have: - Long Term Acute Care (LTAC) experience - Experience in rehab and short-term acute care - Proficiency in biopsychosocial assessments - Ability to coordinate services for home care and equipment following discharge - Skills in documenting pertinent information to facilitate care coordination - Participation in care conference activities - Referrals to rehab and long-term care facilities - Knowledge of adult protective services, child protective services, emergency detention, guardianship, and power of attorney - Experience with discharge planning and advance directives Description: The Medical Social Worker will conduct comprehensive assessments of patients. They will coordinate discharge planning to ensure smooth transitions to home care or facilities. The role includes documenting care provided and interactions with other medical disciplines. The Medical Social Worker will address social, economic, and emotional factors affecting patients. They will supervise staff effectively and efficiently. Knowledge of Medicaid, Medicare, and social service programs is essential. The position may require floating between nearby facilities as needed. This role supports patients and families in navigating healthcare systems and resources. Candidates must be prepared to work on designated holidays.
