Physical Therapist (Home Health) Restore mobility. Relieve pain. Change lives—right at home. Position Summary We’re seeking a licensed Physical Therapist to provide high-quality, in-home therapy services to patients, helping them reduce pain, restore function, and maintain independence. In this role, you’ll work closely with physicians, families, and an interdisciplinary care team to develop and carry out personalized treatment plans. Pay range: 100-115k per year What You’ll Do Evaluate patients’ physical abilities and develop individualized plans of care Provide hands-on therapy to improve mobility, strength, and overall function Educate patients and families on exercises, safety, and use of assistive devices Monitor patient progress and communicate updates to physicians and care team members Document evaluations, progress notes, and discharge summaries in a timely manner Supervise Physical Therapy Assistants as needed Participate in care planning, case conferences, and discharge planning Coordinate visit schedules with clinical leadership and other care providers What We’re Looking For Graduate of an accredited Physical Therapy program Active state Physical Therapist license At least one year of clinical or institutional experience preferred Strong communication, organization, and patient education skills Reliable transportation, valid driver’s license, and required auto insurance Why This Role Matters You’ll play a key role in helping patients recover safely at home while supporting their independence and quality of life—one visit at a time. #J-18808-Ljbffr
