RN Case Manager
Virtual Vocations Inc
Providing comprehensive coordination of services for service members and their families, the full-time RN Case Manager will manage patient care, documentation pathways, and act as a liaison between military and civilian medical agencies in a remote setting. Key responsibilities Conduct assessments, planning, and evaluation of care to promote optimal health and support transitions back to duty or civilian life Coordinate patient care and documentation pathways while ensuring continuity of care across various agencies Review and summarize case facts, rendering medical necessity determinations and resolving technical issues within assigned cases Required qualifications Bachelor's degree with 5-7 years of relevant experience, with an advanced degree preferred Active RN license with a minimum of a two-year nursing degree from an accredited institution At least 2 years of nursing and/or case management experience Certification as a case manager by a recognized organization or the ability to obtain certification within one year of hire Professional oral and written communication skills with various stakeholders, including Military Health System personnel
