Responsibilities
- Provide and document nursing care for homebound patients, including assessment, planning, and intervention in accordance with physician orders and protocols.
- Administer medications and therapies, monitor patient status and responses, and promptly report changes to the care team.
- Collaborate with other healthcare professionals and community resources to coordinate comprehensive home-based care for patients and families.
- Maintain accurate, confidential clinical records and communicate data effectively in verbal and written form.
- Exercise independent judgment within established guidelines and demonstrate critical thinking in urgent or emergency situations.
- Be willing to accept on-call responsibilities and manage scheduling with flexibility to support patient needs.

Requirements
- Massachusetts Licensed Practical Nurse license (must be uploaded with traveler profile) and Massachusetts Temporary Nursing Service Agency license (Nursing Pool) required upfront.
- Active American Heart Association BLS or American Red Cross CPR certification.
- Minimum 1 year of acute nursing care experience; hospital-based experience preferred.
- Experience in home health required; Certified Home Health professional with proficiency in OASIS highly preferred.
- Infusion therapy experience preferred.
- Valid driver’s license and active auto insurance; traveler must have own vehicle and be able to drive in inclement weather.
- Demonstrated ability to work independently in a home setting, maintain patient confidentiality, and communicate clearly with patients, families, and the care team.
