Field-Based Case Manager - Pediatric Behavioral Health

CVS Health is seeking a Field-based Case Management Coordinator to support the OhioRISE program in Northeast Ohio. You will travel to member homes and other locations up to 50% of the time, primarily in Marion, Union, Portage, Geauga, Harrison, Belmont, Monroe, Jefferson, and Carroll counties. Work hours are Monday–Friday, 8:00 a.m.–5:00 p.m., with flexibility for after-hours as needed. Responsibilities include evaluating member needs, coordinating care plans, collaborating with MCOs and #J-18808-Ljbffr CVS Health
