Healthcare Community Liaison Job Summary Cedar Point is seeking a highly motivated and results-driven Healthcare Community Liaison to join our team in Clyde, Ohio . As a Healthcare Community Liaison , you will be responsible for building relationships with referral partners, educating them on our services, and helping patients and families connect with the right level of post-acute care. Responsibilities Build and maintain strong relationships with hospitals, physician offices, and other referral partners to generate referrals and grow market share Develop and execute a territory development plan to increase census and revenue Identify opportunities to expand our services and partnerships throughout the community Cultivate relationships with referral sources, including case managers, discharge planners, and other healthcare professionals Develop and maintain a comprehensive understanding of Sweetwater Care's services and post-acute care options Provide education and training to referral partners on Sweetwater Care's services and benefits Identify and capitalize on opportunities to improve patient outcomes and reduce healthcare costs Requirements Competitive salary A strong background in healthcare sales, business development, or a related field Experience in building and maintaining relationships with referral partners, including hospitals and physician offices Proven track record of success in territory development and census growth Excellent communication, organizational, and problem-solving skills Ability to work independently and as part of a team Strong networking and community outreach skills What We Offer A dynamic and supportive work environment Opportunities for professional growth and development Competitive compensation package #J-18808-Ljbffr Skilled Nursing of Ohio
