RN Care Manager - Full Time

The Care Manager plays a vital role within an interdisciplinary team, assessing, planning, advocating for, and coordinating patient care from admission to discharge to ensure safe post-hospitalization transitions. Responsibilities Responsibilities include overseeing transition planning, managing length of stay, and maintaining high-quality patient experience, safety, and care standards using performance metrics and best practices. Qualifications & Requirements Graduate of an accredited School of Nursing Bachelor's Degree of Science in Nursing Current state nursing license BLS (CPR) certification at hire or within 90 days 2-3 years of acute care hospital experience preferred Care or Utilization Management experience preferred Certification in Case Management is a plus.
