Registered Nurse - Home Health/Hospice

Job Description

We are seeking a skilled Registered Nurse to provide comprehensive nursing care and case management for assigned patients in their residences or at our in-patient Hospice Center. The role involves admitting patients, coordinating services for continuity of care, negotiating needed resources, and ensuring quality clinical outcomes through effective nursing practices and patient/family instruction.

Responsibilities

• Provide skilled nursing care and coordinate services for patients and their families in a residential setting.
• Observe, assess, evaluate, and plan interventions based on patients' physical and psychosocial statuses.
• Oversee care provided by Home Health/Hospice Aides and CNAs, ensuring compliance with care plans.
• Facilitate effective communication and case conferences with interdisciplinary teams and community health agencies.
• Deliver quality care with appropriate symptom management and promote optimal health through education and referrals.
• Maintain accurate patient records in compliance with Agency standards and OASIS requirements.
• Participate in on-call rotation and team scheduling as necessary to meet patient needs.
• Engage in quality improvement activities and attend assigned in-service training.

Required Experience / Certifications / Licensure

• Current RN license in the State of Montana (BSN preferred).
• Valid driver's license, appropriate auto insurance, and reliable transportation.
• 2 years of recent acute care, home care, or hospice experience preferred.
• Clinical skills in venipuncture, catheterization, wound care, and/or parenteral therapy are preferred.

Schedule

Schedule varies, with a typical in-home RN schedule of Monday to Friday, 8 AM to 5 PM with an on-call rotation.
