Job Description
Deliver the right care, at the right time, in the right place as part of Elara Carings home health team. Provide skilled physical therapy services to patients in their homes, helping restore mobility, strength, and independence.
Responsibilities
- Perform assessments and develop individualized care plans for home health patients.
- Provide therapeutic interventions and educate patients and families on exercises and safety.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Document patient progress and maintain accurate clinical records.
Qualifications
- Active Physical Therapist license in the state of Illinois.
- CPR certification (AHA or American Red Cross).
- Minimum 1 year of experience in a healthcare setting.
- Reliable transportation for home visits.
- Strong communication skills and commitment to quality patient care.
